Issue
Dreamweaver menu commands are removed and replaced with new items after installing the "ASP.NET Form Controls" extension. This extension is part of the DevNet Resource Kit (DRK) volume 3. Uninstalling the extension causes all instances of these commands to be deleted from the menus and the corresponding objects to be removed from the Insert bar.

Solution
To restore the Dreamweaver menus and the Insert bar to their original state, download and install the "Restore ASP.NET Defaults" extension (PreASPNetDefaults.mxp). This file will restore the Insert menu, Insert bar and Shared directory structure to the same state they were in before the ASP.NET Form Controls extension was installed.

Note: In order for the new changes to take effect, Dreamweaver must be restarted.

1	Download the "Restore ASP.NET Defaults" extension.
2	Launch the Macromedia Extension Manager and install the "Restore ASP.NET Defaults" extension (PreASPNetDefaults.mxp). Note: This only needs to be done after uninstalling the "ASP.NET Form Controls" extension (ASPNETFormControls.mxp).
3	A warning dialog box may appear while installing this extension, saying that "a newer copy of <name of file> already exists" and asking if you would like to replace it. Five files need to be restored by the extension, so the warning will appear five times. Replacing these files with the older versions is appropriate in this case. The existing files overwritten during this process are: ServerObject-MenuProps.js dwscriptsExtData.js tagDialogsCmn.js TagEditClass.js MMInit.htm

Note: Uninstalling the "Restore ASP.NET Defaults" extension will cause the menus to be removed again.
